P0278 Cylinder 6 Contribution / Balance Fault

P0278 is a diagnostic trouble code (DTC) that means the engine control module (ECM/PCM) has detected an abnormal power contribution or combustion balance from cylinder #6.

In practical terms, the engine computer believes cylinder #6 is not producing the expected amount of power compared with the other cylinders.

This code does not automatically mean that the cylinder #6 fuel injector is faulty. An injector problem is certainly possible, but the same condition can be caused by fuel pressure, injector wiring, ignition, compression, valve operation, air supply, engine timing, or other combustion-related problems.

What Does “Contribution / Balance” Mean?

Each cylinder should contribute a relatively similar amount of torque to the engine.

When a cylinder fires correctly, it produces a small increase in crankshaft speed. The ECM can monitor these changes and use them to estimate whether individual cylinders are contributing normally.

If cylinder #6 consistently produces less torque than expected, the ECM may determine that its contribution is below the expected level and store P0278.

“Balance” refers to how evenly the cylinders are contributing to overall engine operation.

For example, if the other cylinders are operating normally but cylinder #6 is consistently weaker, the ECM may identify a cylinder contribution or balance problem.

P0278 Does Not Identify a Specific Failed Part

This distinction is important when diagnosing the code.

P0278 describes what the engine computer has detected, not necessarily why it happened.

The underlying cause could be:

  • Fuel injector problem

  • Injector wiring or connector

  • Low fuel pressure

  • Fuel contamination

  • Ignition problem on a gasoline engine

  • Low compression

  • Valve or valvetrain problem

  • Camshaft or valve timing problem

  • VVT problem

  • Air intake problem

  • EGR-related issue on applicable engines

  • Engine mechanical damage

Replacing the injector without testing the rest of the system can therefore lead to an unnecessary repair.

Where Is Cylinder #6?

The location of cylinder #6 depends on the specific engine.

There is no universal physical location for cylinder #6 across all manufacturers.

This is particularly important on V6, V8, V10 and V12 engines. Cylinder numbering and bank arrangement can differ considerably.

Always identify cylinder #6 using the manufacturer's service information before testing the injector, spark plug, compression or wiring.

Common Causes of P0278

Fuel injector problem

A cylinder #6 injector that is restricted, contaminated, worn or delivering an incorrect amount of fuel can reduce the cylinder's power contribution.

Possible injector-related causes include:

  • Restricted injector

  • Poor spray pattern

  • Internal injector wear

  • Injector leakage

  • Incorrect injector calibration

  • Incorrect injector coding

  • Fuel contamination

  • Internal electrical fault

On modern diesel engines, injector correction and smooth-running values can provide useful diagnostic information.

Injector wiring or connector problem

The injector may be mechanically healthy while its electrical circuit has a problem.

A loose terminal, corrosion, damaged wire or intermittent connection can affect injector operation.

Engine vibration can make an intermittent wiring problem particularly difficult to reproduce.

Low fuel pressure

Insufficient fuel pressure can prevent the injector from delivering the required amount of fuel.

Possible causes include:

  • Weak fuel pump

  • Restricted fuel filter

  • Fuel pressure regulator problem

  • High-pressure fuel pump problem

  • Fuel pressure control fault

  • Air entering the fuel system on applicable diesel engines

A fuel-pressure problem may affect several cylinders rather than cylinder #6 alone, so the overall pattern of the fault is important.

Ignition problem on gasoline engines

Gasoline engines require reliable ignition to burn the air-fuel mixture.

A weak spark on cylinder #6 can reduce its contribution.

Possible causes include:

  • Worn spark plug

  • Fouled spark plug

  • Defective ignition coil

  • Damaged ignition wiring

  • Poor coil connection

  • Incorrect spark plug

A weak ignition component may work reasonably well at idle but fail under load.

Low compression

A cylinder cannot produce normal power if it cannot maintain adequate compression.

If several cylinders are affected or additional timing-related codes are present, this possibility becomes more important.

Air or EGR-related problem

On some engines, an air-distribution problem or abnormal EGR operation can affect combustion quality.

This is especially relevant on certain diesel engines where EGR and intake-air control have a significant effect on cylinder combustion.

Symptoms of P0278

The severity of the symptoms depends on how much cylinder #6 is underperforming.

Possible symptoms include:

  • Check Engine Light

  • Rough idle

  • Engine vibration

  • Uneven engine operation

  • Hesitation during acceleration

  • Poor throttle response

  • Reduced engine power

  • Increased fuel consumption

  • Misfire

  • Hard starting

  • Stalling

  • Increased emissions

  • Smoke on some diesel engines

A small contribution imbalance may be detected by the ECM before the driver notices a major problem.

A severe imbalance, however, can make the engine shake noticeably.

Can P0278 Cause a Misfire?

Yes.

If cylinder #6 is not producing normal combustion torque, the ECM may also detect a misfire.

For example, an injector that delivers insufficient fuel could cause weak combustion. That weak combustion can then produce both a cylinder contribution/balance fault and a cylinder-specific misfire code.

However, P0278 is not simply another name for a misfire code.

P0278 specifically concerns the cylinder's contribution or balance compared with the expected operating condition.

How to Diagnose P0278

The most effective approach is to determine why cylinder #6 is producing less power, rather than immediately replacing the injector.

1. Scan for all DTCs

Start with a complete scan of the vehicle.

Look for additional codes related to:

  • Misfires

  • Injector circuits

  • Fuel pressure

  • Camshaft position

  • Crankshaft position

  • VVT

  • Air/fuel mixture

  • Low system voltage

Freeze-frame information can also reveal the engine speed, load and temperature when P0278 was detected.

2. Check cylinder contribution data

If the diagnostic scanner supports cylinder balance, smooth-running or contribution measurements, compare cylinder #6 with the other cylinders.

This can help determine whether the problem is isolated to #6 or whether several cylinders are affected.

The acceptable correction or contribution range is manufacturer-specific. There is no single universal value that applies to every engine.

3. Inspect the cylinder #6 injector

Check the injector and its connector carefully.

Look for:

  • Corrosion

  • Loose terminals

  • Damaged pins

  • Oil or fuel contamination

  • Broken connector locks

  • Damaged wiring

The harness should also be inspected for chafing, heat damage and poor connections.

4. Test the injector

Depending on the engine, injector operation can be evaluated through electrical testing, injector balance testing, current waveform analysis or manufacturer-specific diagnostic procedures.

On some systems, swapping the cylinder #6 injector with another cylinder can be a useful diagnostic method.

If the contribution problem moves with the injector, the injector becomes much more likely to be the cause.

Injector coding or calibration may be required after replacement on certain modern diesel systems.

5. Check fuel pressure

Fuel pressure should be measured and compared with the manufacturer's specifications.

It is important to check pressure under the conditions in which the fault occurs. A fuel system that maintains normal pressure at idle may still have a problem under acceleration or high load.

6. Check ignition on gasoline engines

Inspect the spark plug and ignition coil for cylinder #6.

Where appropriate, swapping the coil or spark plug with another cylinder can help determine whether the problem follows the component.

7. Perform a compression test

If the fuel and ignition systems appear normal, compare compression between cylinders.

A significant difference involving cylinder #6 can point toward an internal mechanical problem.

8. Perform a leak-down test if necessary

A leak-down test can help identify where compression is being lost.

Air escaping through the intake indicates a possible intake-valve problem.

Air escaping through the exhaust indicates a possible exhaust-valve problem.

Air escaping through the crankcase can point toward piston rings or piston damage.

9. Check timing and VVT

If mechanical problems are suspected, inspect camshaft timing, crankshaft/camshaft synchronization and VVT operation.

This is especially important when P0278 appears with camshaft or crankshaft-related DTCs.

10. Consider the ECM last

An ECM/PCM problem is possible, but it should normally be considered only after the injector, wiring, fuel system, ignition system and mechanical condition have been properly tested.

P0278 vs Injector Circuit Codes

P0278 should not be confused with a cylinder #6 injector circuit fault.

An injector circuit code indicates that the ECM has detected an electrical problem with the injector circuit.

P0278 indicates a cylinder contribution/balance problem.

This distinction is useful because a cylinder can have poor contribution even when the injector circuit is electrically normal.

For example, cylinder #6 could have:

  • Normal injector electrical operation

  • Normal injector wiring

  • Normal fuel pressure

but still have poor contribution because of low compression or a valve problem.

P0278 vs P0277

These codes concern cylinder #6 but describe different types of faults.

P0277 – Cylinder 6 Injector Circuit High indicates an abnormally high electrical condition in the cylinder #6 injector circuit.

P0278 – Cylinder 6 Contribution / Balance Fault indicates that cylinder #6 is not contributing to engine operation as expected.

One is primarily an injector circuit electrical diagnosis, while the other is a cylinder performance/contribution diagnosis.

P0278 vs P0279

P0278 and P0279 are also easy to confuse.

P0278 – Cylinder 6 Contribution / Balance Fault

This concerns the amount of power or combustion contribution from cylinder #6.

P0279 – Cylinder 7 Injector Circuit Low

This concerns an abnormally low electrical condition in the cylinder #7 injector circuit.

They affect different cylinders and different aspects of engine operation.

Is P0278 Serious?

The seriousness depends on the severity of the cylinder imbalance.

If the engine is running smoothly and the Check Engine Light is steady, the vehicle may be able to reach a repair facility for diagnosis.

However, continued driving is not advisable when there is:

  • Severe engine vibration

  • Significant power loss

  • Flashing Check Engine Light

  • Heavy misfire

  • Stalling

  • Strong fuel smell

  • Excessive smoke

  • Abnormal engine noise

A severe misfire on a gasoline engine can allow unburned fuel to enter the exhaust system and overheat the catalytic converter.

On diesel engines, a serious injector or combustion problem can increase smoke and emissions and may contribute to further engine or aftertreatment problems.

A Practical Diagnostic Approach

P0278 is best approached by asking a simple question:

Why is cylinder #6 producing less power than the other cylinders?

Start with the data from the ECM, then compare cylinder contribution. After that, check the injector and its wiring, verify fuel pressure, inspect the ignition system on gasoline engines, and move toward compression, valvetrain and timing checks if the basic systems are healthy.

The code itself does not condemn the injector or any other single component. A proper diagnosis should identify whether the problem is fuel-related, electrical, ignition-related, mechanical or timing-related before parts are replaced.
